If your organization has received a grant from the Scanlan Foundation for Session 2019A or later and you are unsure if you have submitted all of the required paperwork to remain eligible for future grants, please download the corresponding form below, print it out, scan it in, and submit, or re-submit, the form(s) by email to grants@scanlanfoundation.org.

Scanlan Foundation IRS Receipt Forms

Recipients of grant awards from the Scanlan Foundation must submit a Scanlan Foundation IRS Receipt Form for each award received as of 2019A in order to remain eligible for future grant awards.

Scanlan Foundation Tuition Assistance Report Forms

Additionally, high schools who have received grant awards from the Scanlan Foundation must submit a Scanlan Foundation Tuition Assistance Report Form for each grant received as of 2019B in order to remain eligible for future grant awards.
